Health Fair & Community Event Participation l Request Form ![]()
Madison County Health Department provides health education and promotion on a number of health topics. If you would like to have us participate in a health fair, provide a presentation or training, or participate in an event or other activity your organization is holding, please complete our request form at least one month in advance. Once your request has been received, we will reply to you within 2 weeks.

The Health Promotion Division strives to engage the community in order to positively impact the health decisions and the environments of individuals, families and communities.
Our Mission:
Using evidence-based research and practices, the Health Promotion Division designs and develops strategies to communicate accurate health information and deliver quality programming for residents to increase knowledge and skills and to positively influence policy and practices, improving health and quality of life.
